If you wish to make a verification payment online using a credit card, it will cost $35 per practitioner. Verification payments are processed through the online system ($35/practitioner) with a credit card. ABFAS accepts Visa, Mastercard, American Express, and Discover.
If you are unable to submit online, please download the Individual Status Verification Payment Form. The fee will be $42 per practitioner. Mail your check to PO BOX 889405, Los Angeles, CA 90088-9405, or fax credit card information to 415.553.7801.
To purchase a block of 15 verifications ($35 each) for $525, download the Block Verifications Payment Form to add funds to your account. Fax the credit card payment information to 415.553.7801, or mail check payment to PO BOX 889405, Los Angeles, CA 90088-9405.
